If one wants to shunt mod their 3090 prior to applying the waterblock, must they replace the resistors entirely, or can they stack new resistors atop the original ones? What about on the backplate side too?

Sorry that we do not understand what you meant.
The 3090FE water block has clearance for stacked shunt resistors or not,

It depends on the space of your motherboard and case.
But if you want to make SLI with two 3090 FE water blocks, you can use the fittings or the bridge, such as BP-VGAB14AC-CL to replace two links of the water blocks. The bridge depends on the distance of the slots on your motherboard.
